Physical therapist · Dallas–Fort Worth
Jake Thomas, PT, DPT is the clinician behind Physical Therapy 365. He provides one-on-one mobile outpatient physical therapy for orthopedic, post-surgical, neurological, balance, and mobility needs across selected Dallas–Fort Worth communities.
Credentials
Jake earned his Doctor of Physical Therapy degree from the University of St. Augustine for Health Sciences, Austin Campus, in 2016. His individual NPI record is active in the National Plan and Provider Enumeration System under NPI 1992244537.
The credentials “PT, DPT” identify Jake as a physical therapist with a Doctor of Physical Therapy degree. His work through Physical Therapy 365 is centered on direct, individualized care and practical movement goals.
Rather than dividing a visit among multiple patients, aides, or rotating providers, the PT365 model is designed around one clinician, one patient, and a clear plan that can be applied in the patient’s own environment.
Clinical experience
Jake has practiced as a physical therapist since earning his DPT in 2016. His experience includes mobile orthopedic outpatient care and rehabilitation for patients with orthopedic, post-surgical, neurological, balance, gait, and mobility needs.
Physical Therapy 365 applies that experience through one-on-one visits across Dallas–Fort Worth. Each plan is based on the patient’s examination, medical context, precautions, environment, and goals rather than a standard protocol applied to everyone.
Clinical focus
Jake’s clinical work addresses movement problems that affect recovery, independence, confidence, and participation in everyday life.
Evaluation and treatment for joint, muscle, tendon, spine, gait, and activity-related problems.
Progressive rehabilitation after orthopedic procedures, including knee and hip replacement when appropriate.
Function-focused care for selected neurological, coordination, gait, and mobility limitations.
Strength, balance, walking, transfer, assistive-device, and fall-risk considerations in the patient’s real environment.
Skilled outpatient physical therapy delivered in homes and other appropriate preferred locations.
Plain-language explanations, realistic expectations, and a home plan designed to be understood and used.
Treatment philosophy
Physical therapy should connect clinical findings to the tasks a patient wants or needs to do. That begins with listening, examining the relevant movement problem, and identifying reasonable priorities together.
Treatment may include exercise, movement practice, manual techniques when appropriate, balance or gait training, education, activity modification, and a home program. The exact approach depends on the evaluation, health history, precautions, symptoms, and goals.
Jake’s role is also to recognize when a presentation needs medical evaluation, coordination with another clinician, or a change in the rehabilitation plan. Physical therapy is one part of healthcare, not a substitute for urgent or specialist care when that is needed.
