Post-surgical PT
Progressive rehabilitation after knee replacement, hip replacement, and other orthopedic procedures when mobile outpatient care is appropriate.
Highland Park, Texas · Mobile outpatient care
Physical Therapy 365 brings one-on-one outpatient physical therapy to Highland Park patients. Jake Thomas, PT, DPT treats orthopedic, post-surgical, balance, mobility, and selected neurological needs in the setting where patients move through daily life.
Care that fits the community
For Highland Park residents, a clinic visit can add driving, parking, waiting, and schedule disruption to an already busy recovery. Mobile PT removes that commute while keeping the visit focused on skilled outpatient rehabilitation.
Home-based sessions can also make treatment more specific. Instead of describing difficulty on a staircase, getting out of a favorite chair, or walking along the block, the patient and therapist can assess and practice the actual task when it is safe to do so.
Patients generally do not need to be homebound because this is mobile outpatient physical therapy, not a home health episode. Learn more about how mobile outpatient PT works.

What to expect
The first visit includes a review of the patient’s main concern, medical and surgical history, precautions, medications relevant to movement, and goals. The physical examination may include range of motion, strength, balance, walking, transfers, stairs, and other task-specific measures.
Jake then explains the findings and recommends a plan. Treatment can include therapeutic exercise, movement practice, balance or gait training, manual techniques when appropriate, education, and a home program. Progress is reassessed and the plan changes as the patient’s needs change.
