When Should Physical Therapy Start After Knee Replacement?
Physical therapist–guided movement commonly begins within the first day after uncomplicated knee replacement and before hospital discharge. The start of home health or outpatient PT depends on the surgeon’s protocol, medical stability, and discharge plan.
Why does rehabilitation begin early?
Early rehabilitation helps the patient learn safe transfers, begin walking with the appropriate device, activate key muscles, and understand precautions and a basic home plan. It also allows the care team to determine what assistance or setting is needed after discharge.
The APTA total knee arthroplasty clinical practice guideline recommends starting physical therapist management within 24 hours of surgery and before discharge for typical patients. That recommendation does not replace individual surgeon instructions or medical judgment.
When does outpatient PT begin?
Some patients transition directly to outpatient PT, including mobile outpatient care. Others first receive home health or spend time in a rehabilitation setting. The appropriate sequence depends on medical needs, mobility, support, insurance, transportation, and surgeon preference.
Contacting an outpatient provider before surgery can make the transition smoother, but the actual first visit should match the postoperative plan.
What are the first outpatient priorities?
The therapist reviews the operation, wound and medical instructions, symptoms, assistive device, motion, swelling, muscle performance, walking, transfers, and stairs. Priorities are individualized rather than based on forcing a particular exercise list.

Can PT start too late?
Unnecessary delays may allow avoidable stiffness, weakness, or mobility limitations, but the safe timing depends on medical and surgical factors. Contact the surgeon if the plan is unclear.
